Cost analysis by type of surgery at the Pediatric Cardiac Surgery Center at Fann University Hospital

Abstract

Introduction: A specialized center dedicated to cardio pediatric surgery was established at Fann University Hospital in 2018 to improve the management of children with heart disease in Senegal and the sub-region. We performed a cost analysis of the surgeries. The objective of the study was to determine the cost per type of surgery and to propose a flat rate accessible to patients. Method: Quantitative and qualitative data on the performed surgeries in the center were collected retrospectively to estimate their cost. These included the types of surgery performed, consumables, drugs, human resources required, and the average duration of the surgeries... Data analysis was performed with Sphynx software. For each type of surgery, direct and indirect medical costs were estimated. The estimated average cost was subsequently compared with the flat rate initially applied before this study. Results: Depending on the type of surgery, the cost ranged from 1,777,343 FCFA (2,709 €) for a closed-heart surgery without implant to 4,700,121 FCFA (7,164 €) for surgeries with two biological prostheses and a ring. The initial fixed price of 3.500.000 FCFA (5.335 €) covers all charges of the cardio pedriatric surgery center. Conclusion: This study made it possible to estimate the cost of each surgical intervention performed at the CUOMO Cardio pediatric Center (CCPC). The costs of valve replacement are higher than the applied flat rate. A subsidy, from the partners or from the State, is therefore necessary to maintain the flat rate at the same price.
