Frequency and associated factors with overweight/obesity among hypertensive adults at primary care in Bobo–Dioulasso, Burkina Faso

Abstract

Introduction: In urban Burkina Faso, a quarter of adult population live with hypertension, which is among the top 10 most frequent complaints in primary health care centres (PHCCs). This study aimed to describe the frequency and associated factors with overweight/obesity among hypertensive adults, at PHCCs in Bobo-Dioulasso. Methods: A cross-sectional survey was conducted in a sample of 380 hypertensive adults from January to February 2022, in 20 public PHCCs, in Bobo-Dioulasso. Socio-demographic and economic parameters, data on physical measurements (anthropometric and blood pressure) and lifestyle practice were collected. Descriptive, bivariate and multivariable analyses were performed. Results: The participants’ mean age was 56.2±10.4 years, the rate of excess weight was 47.6% and uncontrolled hypertension was more frequent in this group (76.8% in participants with excess body weight vs. 67.8% in the others, p=0.052). In logistic regression , the significant associated factors with excess weight were high annual income exceeding the median (aOR=2.0, 95% CI: 1.3 – 3.2), high direct costs of management exceeding the 4th quartile (aOR=3.2, 1.6 – 6.4), regular coffee consumption (aOR=1.9, 95% CI: 1.2 – 3.1), excessive salt consumption exceeding two tablespoons (aOR=1.9, 95% CI: 1.1 – 3.3), tobacco use (aOR=0.4, 95% CI: 0,2 – 0,9) and uncontrolled hypertension (aOR=1.9, 95% CI: 1.1 – 3.3). Conclusion: Excess body weight in hypertensive patients is common in primary care, and has a negative influence on the control of hypertension. At this care level, appropriate intervention strategies for weight control need to be found.
