Impact of Therapeutic Education of Patients on the observance of seropositive subjects, under antiretroviral treatment at the University Hospital of Point G in Bamako

Abstract
Introduction: The advent of antiretroviral therapy has greatly contributed to improving the quality of life of patients. However, it is not uncommon to observe after therapeutic gain in patients who have fallen into a cycle of non-compliance, a corollary of the advent of mutants resistant to ARV molecules. Objective: Our work aimed to assess the benefit of therapeutic education on ARV adherence and recovery in PLHIV considered to be non-compliant. Patients and Method: This was a quantitative and prospective study in non-observant patients with treatment failure. The work consisted of including in a regular TVE program conducted over a period of 6 months. The variables to measure PTE were viral load and CD4 6 months after treatment reinforcement. Results: A total of 50 patients were included, 54% of whom were women. The mean age was 38± 8.04 years (23y and 78y). The mean duration of ARV treatment was between 2 and 12 years. 54% were on a 2nd line regimen. All patients were in virological failure (100% detectable CV). However, 94% of patients had undetectable CV after 6 months of boosting. Median CD4 count increased from 216 to 313 cells/mm3.
Conclusion: TPE is a strategy whose application brings a considerable benefit in limiting the emergence of resistant mutants in PLWHIV.
