Thrombocytopenia in HIV: epidemiological, clinical and paraclinical aspects at the CHUSS of Bobo-Dioulasso

Abstract

Thrombocytopenia is a common condition during HIV infection and can occur independently of other cytopenias and at all stages of infection. The objective of this work was to describe the epidemiological, clinical and biological aspects of thrombocytopenia during HIV infection. Methods: This was a retrospective longitudinal study on the cohort of HIV-infected children followed at the pediatric day hospital of the Bobo-Dioulasso CHUSS over a period of 10 years. Results: out of a total of 404 children followed, 43 had thrombocytopenia, i.e. a prevalence of 10.6%. The mean age was 3.08 ±2.2 years, the sex ratio was 0.8. All patients were infected with HIV1. Polyadenopathy was the most frequent clinical sign with 34.9%. Isolated hemorrhage such as epistaxis and/or gingival bleeding was present in 4.7%. Thrombocytopenia was grade 3 in 4.6% and grade 4 in 2.3%. The CD4 count was on average 1023 ± 610 cells/mm3, the average viral load was 509 ± 1040 copies/µl. Thrombocytopenia was more common during ARV treatment with a frequency of 88.4% compared to 11.6% without treatment. Conclusion: The prevalence of thrombocytopenia in HIV is high. It was more common in patients on treatment compared to patients naive to antiretroviral treatment
