Pyogenic liver abscess : clinical, morphological and therapeutic aspects carried out on 19 cases

Abstract

Objective: To study the epidemiological, clinical, morphological and therapeutic aspects and to identify the most harmful germs responsible for pyogenic liver abscess. Design: A retrospective descriptive study spanning from January 2011 to August 2013 in the department of infectious diseases. Were included all patients with liver abscess with ultrasound and / or CT. Results: We collected 19 cases of liver abscess during the study period. The average age of patients was 53 ± 7 years with a slight female predominance. The classic triad of Fontan: painful hepatomegaly and fever was found in 14 cases. The biological inflammatory syndrome was present in all patients. All patients underwent an abdominal ultrasound and abdominal CT was performed in 11 patients. Abscesses were unique in 8 cases, localized on the right lobe in 7 patients. The germs were isolated in 6 patients, it was an Enterobacter cloacae, an Escherichia coli, and anaerobic gram-negative bacilli (Bacteroides fragilis, eubacteruim) isolated in samples abscesses. Klebsiella Pneumoniae one was isolated from three blood cultures . The combination of antibiotics was prescribed for all patients based on beta-lactam + imidasolé in 6 cases and beta-lactam + aminoglycoside + imidazole in 13 cases. The open surgery was performed in 3 patients. The evolution was marked by the complete cure in 15 cases, recurrence in 1 case and death in 3 cases.
