Stop frequenting Teaching Hospital of Point G in Bamako by patient on antiretroviral therapy: A longitudinal Analysis for Risk Factors

Abstract

OBJECTIVE : Determine the ratio of patients in stop frequenting Teaching University Hospital of Point G after starting their antiretroviral therapy (ART) in this hospital and identify some risk factors of this stop frequenting. METHODS: We analyzed pharmacy refill records of adult patients initiating ART between July 19, 2004 and July 31, 2009 at Teaching University Hospital of Point G. Patients were classified as in stop frequenting if they did not return 90 days from their expected visit. We Revue Malienne d'Infectiologie et de Microbiologie 2014, Tome 4 Page 22 used Kaplan Meier method to determine the ratio of patients in stop frequenting and we identified risk factors associated with stop frequenting using Cox regression. RESULTS: Of 3042 patients initiating ART, just one death was notified, 5 transfers were notified and 79.5% of patients were in stop frequenting of Teaching University Hospital of Point G. Far residence to Teaching University Hospital of Point G (reference=Bamako, Out of Bamako P<0,001), male (P<0,001), profession (reference= civil servant, unemployed P<0,001, worker P<0,001, other P=0,005), therapeutic change (reference= patient without change, change because of tuberculosis P=0,048, change because of drug breaking P=0,026), patients aged of de 50 years old and more (reference=40-49 years old P=0,021) and year of initiating ART (reference=2004, 2005 P=0,001, 2006 P<0,001, 2007 P=0,017, 2008 P=0,388, 2009 P=0,068) were associated with high risk to stop frequenting Teaching University Hospital of Point G after initiating on ART. CONCLUSION: Our study pinpointed a high ratio of stop frequenting Teaching University Hospital of Point G by patient after initiating ART. We suggest to improve notification by creating a unique refill records that should be accessible for all clinicians who work on ART at Teaching University Hospital of Point. In additio , we suggest to initiate a study in order to understand the becoming of patients in stop frequenting at Teaching University Hospital of Point G.
